Yes, proof of purchase tabs from both the 2-disc and 4-disc editions, and a copy of the purchase receipt for the 4-disc edition. (And the original coupon that comes in the 4-disc set.)
Without a significant amount of footage added, I don't see the need for an upgrade. With so little stuff added back, all having very little apprarent effect on the complete film, I'd think that's what a deleted scenes section would be for.
I'm quite happy with the 2-disc collector's edition.
No booklet in mine, either. It's nice they thought it was important enough to display in the picture and advertise as a special feature, yet it seems like they've left them out of many of the sets. Hope Disney is ready to start sending these out....
If you're missing the booklet, just call Disney (1-800-723-4763) and they'll be glad to send you one. I got the booklet in mine, but I've called them for other things before and they've always been very helpful and easy to work with to get the issue resolved quickly.
