This is going to ebay but I thought I'd offer it here first.
It's a Extron SW4 AR. It has 4 component/RGBS inputs and 1 output. It has 300Mhz of bandwidth, which means it will switch HD stuff with no loss.
It has bnc style connectors, so if your running rca type component cables, you have to buy converters. I bought mine from radio shack.
The serial number is etched in the top, but other than that it's in great shape.
