The only other movie that had this type of effect on me was "Cannibal Holocaust" - only because it felt like you were watching a snuff film.
Although the special effects in this clip are beyond anything I've seen lately, this would NEVER get into a theater as-is. Not without a NR or X rating.
Which brings me to this question, why do studios shy away from NR ratings? Is there a rating that is stronger than a R (besides X which is too familiar with porn movies)?

The short answer is that many media outlets will not accept advertising for unrated films. Perhaps more importantly, the more the audience is limited by the rating (or lack of one), the less the movie makes in theaters. Which is why all the studios are trying to keep their movies at PG-13.
NC-17. It was supposed to provide the limitations of the "X" rating without the porn stigma. It failed, which is why you seldom see it anymore. The current preference is to release such films unrated (as was the case, for example, with Requiem for a Dream).

Yeah. I recogized the clip immediately. I don't remember the name of the movie but it came with the recent ICP cd. It had a dvd with it. On it was this wierd-ass movie. That decap scene was awesome, though.
