Can't answer your question directly but let me say that inserts were something that could seperate a bootleg from a genuine copy. Inserts were the final, loving touch to your movie package. From Citizen Kane to Tales From The Darkside, no matter, there is no excuse to leave out the insert. Needless to say, this wretched business has put out the flames of my passion for DVD.
My favorite insert is the one in Adaptation. If more people saw that, there'd be lots less "where's the insert" questions.
I really only enjoy inserts when they're a booklet, or at least more than just the chapter listing...chapter listing is kinda useless unless you're watching the DVD, and it's almost always on the menu anyway.
