Andy, having done 250 of mine, I'd say that 10% have spine issues. What I usually did then was just download custom artwork or a scan and use photoshop to edit it. The newer cases that are the same size as a standard DVD single that hold 5 are a god send.
Its a Comcast HD-DVR hooked into the TV through component. If the DVR is completely unhooked from the TV and the DVD player plugged in it still happens so I don't think it's the coax I've got a new power strip here that I'm going to try and hooked just the TV, DVR into it and see how that goes.
