Does anyone know what companies actually make the DVD deluxe style packaging for the studios? For example, who actually manufactured the Fight Club or Tombstone DVD slip cases and panels, etc..

The stuff for Warner titles is done by thier company Ivy Hill http://www.ivyhill-wms.com/Ihwms2/home.html
They also make the dreaded Snapper case. This is who you contact to get replacement Snappers when the come broken.
