    The first thing to try is always to disconnect the power (don't just turn the unit off) for several minutes. 30 minutes or more isn't a bad idea. Then plug it back in and see how it goes.

Manufacturers don't tend to spell things out, but just because the volume control is digitally controlled doesn't necessarily mean it's in the digital domain.     If the cable box has a digital audio output, I'd connect that to the receiver, then use that connection when you use the receiver, instead of passing it through the TV. Volume controls are basically all electronically controlled these days. The dial typically doesn't turn.

    Probably the best most of us can do is guess... I've never used pass-through, but it has to be configured in the receiver's setup menu. I seem to recall typical options being "Last Input" or a default one. So, you'd want to set it to the Cable input. As far as it switching to the TV input...

    Yeah, best to ask first. For decades Pioneer was a giant, then several years ago they virtually vanished. They're back under the umbrella of Onkyo, so they should be solid.
