I prefer the original Sega unit.
It's a bit weaker than the third party units, but I've never had reliability problems with it. I had the madcatz one and it would vibrate like a banshee but then stop working until I rebooted the system.
As with Playstation memory cards, I usually only purchase first party accessories because they seem to be the most reliable.

Interact made the tremor pack. That is the one that I have, and it does a great job of rumbling, but I had to brace it with four pieces of folded paper packed in tight between the tremor pack and the edges of the socket on the controller where the pack plugs in. Otherwise, the rattling noise was really annoying.
I have the Performance rumble packs in each of my four controllers, and they all fit and rumble just fine. They were $4.99 each at one of the local Electronics Boutique locations.

Yeah, i have the TremorPak PLUS! That also inlcudes 200 blocks of memory... It rumbles alot, but its really like messy, but still, alot...
