Apparently, Kino couldn't put both cuts on the disc so they polled Facebook users as to which cut they wanted. Theatrical won.

I believe it came out in Italy at the end of 1966 and came to the U.S. in early 1968.

I'm really happy about that. The extended cut is interesting, but the added scenes really derail the film IMO.
