Actually, I know of one case where this is true.
During The Last Temptation of Christ, Willem Dafoe was required to be crucified in one scene; well, during this scene, Dafoe was also required to be completely nude. Apparently, in some shots a certain...appendage kept popping up, so to speak. So they had to tape "it" down.
Reportedly, a pair of female assistants offered to do the task, but Dafoe said he'd rather do it himself. Though I'm not sure that part is true.

I've read that male porn stars often have to get "worked on" by off camera assistants prior to doing a scene, so simply doing a sexual scene in front of the camera isn't necessarily sufficient to get a person aroused.

I read this from an faq source that the reason some people, especially those in the porn world, do not get aroused in said such scenes is because of the sheer number of people surrounding the set. It's often been noted that when new guys enter the porn world thinking its the greatest job in the world and when it's time to perform, they get surprised by the sheer number of people around the set, including those who aren't even a part of the movie. This sort of thing also happens in regular movies. But of course, there's also that thing about professionalism, acting, whatnot...

I read or saw an interview with a male porn star a year or so ago and remember him talking about the "fluffer" rumor. He said that they were pretty much just that, a rumor, and to his own dismay had never worked on any movie where there was one.

A few years ago I had the SPICE channel for a month or two, before realizing how amazingly expensive it is to get. Anyway, the best shows weren't the movies, they were the "behind the scenes" footage shown between movies. Really, um, interesting stuff goes on behind the camera. There are a few adult "blooper" tapes out there for sale/rent if anyone's interested in that.

I seem to remember a story about the movie Tattoo (1981)
where Bruce Dern & Maud Adams were doing it real. The story
also goes that when Dern was asked if it really happend he said yes, but Adams said it was a lie.
Anyone else remember this? (yeah, I know, I must be getting old)
