I sent an e-mail last Thursday and received this response yesterday. I actually received an e-mail from Amazon about the issue and forwarded that to Sony.

I received my replacement extras disc for LoA 3 days ago! That is good, quick service. I expect some of the early reviews will be revised a bit with the arrival of this edit to the package.

I received my replacement extras disc last week by Fedex. I live in New Zealand
so that is excellent customer service.
