When I am by myself, always reference. And when my wife is watch with me, I have to go -5 - -10 below. Sometimes I will sneak the volume up slowly as the movie progresses and her ears adjust. 
I also have a couple friends that are HT nuts, but can't afford a HT, so they come by and ask me to crank it to ref once and a while. Gotta please the fans.
Still remeber trying to watch LOTR the first relese at reference. Good god I thought my head was going to explode, had to turn it down about 10 mins in. Only to later find out, that the disc was burned hot. Not a fun experience.
And on 1 very rare ocasion, I had a bud come by who is into house music in a big way and is used to hearing it in the clubs. So i poped in a disc and threw it into 2 channel, and let her rip. We hit 10db above reference before I gave in. Never do that again. Ouch.
