Free DVD in JP Trilogy? (1 Viewer)

Just wondering...there was no form of any sort in my boxset. Or it is just limited to those who buy JP III only?

I didn't get either form. Mine was just JP3.

It should be in the JP3 case. I picked up a single copy of JP3, and my form was there.
