A few months ago, I got a DirecTivo along with a triple LNB dish. Now I want to add another DirecTivo - can I run 4 coax cables from the triple LNB w/o getting some kind of adapter or something? Thanks a lot for any help.

The current "phase 3" dish from DirecTV can provide satellite output to four devices. If you have two TiVos that use two each, you are fine. You cannot "split" a satellite cable to try to serve two satellite receivers with only one cable. If you will need more than four total satellite outputs, you will need to use an additional multiswitch.
