Dead Zone Season One Unaired Pilot. Where to get "bouce back" card? (1 Viewer)

I recently picked Season One and Two of the Dead Zone during the DeepDiscountDVD sale. There is a space in the Season One Set for the unaired pilot. You can get it if you send in what they refer to as a "bounce back" card. My set unfortunately didn't come with one. Does anyone have one and could you email me the address from it so I can write Lions Gate or, better yet, anyone know where I could get one?

Some members over at DVD Talk claim that they were able to get it at no charge by calling 310-255-3746 . I haven't done it yet myself, but I plan on calling later this week.
