Can anyone help me get a refund for Next Gen magazine? (1 Viewer)

I have a subscription for Next Gen Magazine through Feb. 2003, but they are out of business. I recently got PSM magazine instead, and they said I could keep that magazine to fill out my subscription or get a refund. The problem is, I lost the sheet that had the refund adress and other info on it. If anyone else has this, could you please share it with me.
