Anyone have e-mail address for Artisan? I'm need the slipcase for Rambo 3-pk (1 Viewer)

I purchased the Rambo 3-pk a few months ago from Columbia House a few months ago, but it didn't come with the slipcase.
I checked the Artisan web site but could not find a way to contact them.
Any info or alternate suggestions would be greatly appreciated. Thank.
