Hopefully someone knows what the problem is here, has encountered it, might have an idea what I've done wrong or may have missed.

I just got my Onkyo 606 yesterday and set it up, connecting to Polk RM6750 set (sub connected: L/R front to sub then to L/R fronts, not pre-out -- option #2 in Polk manual) - ran the Audessey setup

DISH HD DVR (722) connected to CBL/SAT via HDMI 3; with HDMI out to the TV. Components connected to a Philips surge protector boasting "Pure Power Technology".

Watching DISH programming, Star Wars DVD, etc last night was fine. (Great sound, etc -- even my wife, who doesn't have much of an ear for this stuff, commented positively)

This morning I turned on the 606, DISH DVR and TV ... no sound from any of the speakers -- volume to 25. Switched to tuner - got sound; then back to CBL/SAT - got sound.

A few minutes later, pop in the speakers and no sound. Switched to tuner again, got sound; then back to SAT and all was ok again. This happened a few times over about 10 minutes then no problem for the remainder of the time I was watching (1 - 2 hours).

Then, this evening, the same thing happened. (This time, I checked the Listening Mode setup for the SAT on the 606 ... set everything to "Direct"). Cut out 2-3 times, where I just hit the "Setup" button, then returned without making changes and sound returned.

I'm no expert but I would say you have a defective receiver or bad HDMI sat output. It doesn't say in your post whether you have anything else hooked up via HDMI that has the same issue or not. You can try a different HDMI input connection on the receiver to see if the problem persists and try another HDMI component to HDMI3 if you have one. If it does try hooking it up via component/digital audio if that's an option to see if that corrects it. At least then you'll know it's an HDMI issue. There's always the possibility of a bad cable.

Why are you using "option 2"? Connect the sub to the sub out on the 606 and connect the L/R to the 606. The receiver will handle the crossovers better than your sub. Plus, you are missing the biggest piece of the Audyssey setup.

Get your speakers properly connected first, then we can see if we can fix your problems (if they still exist).

I'm thinking defective receiver, but I'm not ruling out other issues. (When I first turned on the receiver today it took about 4-5 minutes before I got sound from the receiver - whether from "Tuner", "GAME", "DVD", or "SAT").

SAT is the only HDMI connection I have coming in - then have HDMI out carrying the SAT, GAME and DVD. I've got a Wii connected to GAME via component cables, and an older DVD which I have connected via S-Video and audio cables.

I'll try your suggestion of a different HDMI in; or component if problems persist.

Something I forgot to mention is that I do get video when there is no audio, or an audio dropout...

I used "option 2" for 2 reasons. First, I'd read where others had gotten better bass sound with this option; and second because I didn't have possession of a y-cable to make the hookup.

I bought my mother a Samsung BluRay player that I hooked up via HDMI. For no reason at all the audio suddenly went out but you still had video. I confirmed through troubleshooting that it was the HDMI out of the player. So it is possible to have video without audio, and I'm sure vice versa, via HDMI.

Update ... after more troubleshooting, and more time dealing with the issue (now also having some video dropout), I've come to the conclusion that the receiver is defective. There are times when the audio and/or video drops out, that even the "tuner" setting will produce no sound (so no cabling is involved).
