    I'm not sure that's exclusive to the fancy stores. The only place I reliably see extended sizes is at Walmart and maybe Kohl's. I don't often see any at Target, TJ Maxx, Marshall's, or Costco (they say to "go online" for extended sizes).

    I've not heard of that. My mother and step-father have had several teeth removed over the years. They've never had any replacements or implants and have never mentioned any issues with their jaw.

    These days, at least around here, you'd probably have to pick whichever one will accept you as a new patient and/or can get you into the office reasonably quickly. Many don't seem to be accepting new patients or it's a wait of several months to get an appointment.

    I've not done this, but I'd expect you will need to provide some sort of proof of ownership. But from what others have reported, it's a relatively stress-free process and I don't think you need to return the discs.
