I would look at something from Pioneer, as there have been very few cases of them not playing discs and seem to be very reliable. I have a Elite DV-C36 5 disc changer and it has never once not played a disc.
I would venture to say the first problem you mentioned was caused by a scratch on the disc. This would explain why it only does it on one side. I doubt it has any connection to a layer change.
