voice syncronization issue (1 Viewer)

I have the items listed below. But I frequently have the issue where the video/audio don't align. This is always video lagging behind audio. I have set my ancient reciever to max delay, but still have issues.

Soooo, if I get another reciever to fix this, and only need 5.1 and the stuff below (+xbox 360 ) what is the most cost effective reciever to accomplish this.

At one time I was looking at the onkyo 506, but I am not sure this is cost effective anymore.

How are you hooked up, HDMI? And is it only on the Blue-ray or is it on the cable box? Now I have ran into this before on DirectTV or Time Warner( Cable Box ) with running HDMI only. If I ran HDMI for picture only and then did the digital out for audio ( coaxial or toslink ) to the AVR,the audio would sync better.
