How are subtitles created for TV show DVDs?

I have checked the article on subtitles at Wikipedia, and it didn't answer this question.

Does someone sit there with a script after the fact and type out all the dialogue so it can be put on the DVD? (Maybe the information was already created for closed caption information?)

Once the English version is created, presumably the foreign subtitles are made based on that?

In the recently-released 7th Season of Hawaii Five-O, the word "bastard" appears in one show which was a big surprise to me and others (this word was heard loud and clear as uttered by Jack Lord in an 11th season show).

This word is kind of muffled, since a fight is going on, but if you listen carefully, the character really is saying this word. So how did someone know this?
