MS Word template for DVD Cover

I buy my DVD-R's in the standard case (it's 5.25" wide, 7.5" tall) and has a sort of clear sleeve on the outside that allows a label to be inserted.

Does anybody have a template for MS Word that I could use to print on the insert? or at least to print on a page that could be cut and used as an insert.

I started creating one and don't have it just right. After several hours of frustration, I figured I'd ask out here and see if anybody has already done it.

Did you want to create a cover for your dvd's (front, spine, & back) or an insert? If you want a template for a cover, you can find jpeg templates at DVDCoverArt. (Sorry not a Word template)

You may have to register but templates are free for download.
