I made my own for a heavy / big center speaker. See my link. Since it's hard to see the details from the photos I've posted, I could take a few more and post them for you, especially considering I have a prototype in my basement that I can take apart. Let me know.
BTW, my stand does have a built-in tilt to it, and with the use of screw-in feet, add'l (albeit small) tilt flexibility exists. On the negative side, it was not made very sturdy, i.e., it doesn't take much to knock it over (but it hasn't yet; I keep the kids away).
