The episode itself was kind of "blah", but it teetered into complete insanity and to me, the moment that broke all interest in the show.
Seattle Grace, feeling economic pressure, decides to "merge" hospitals with Seattle Mercy, a teaching hospital (university tied). Seattle Mercy has been raised in the past as an outlet for those who wanted to leave and teach, but when they played at at the end of "economic times are tough, we're going to merge hospitals" I about fell out of my chair.
I've always wondered who consults on these shows. Now I realize it maybe nobody.
This show pretty well jumped the shark last season, but this is as though fonzi drove back to the shark tank and decided to swim his way back across.

The "background" music was overbearing.
Well, there was plenty of drama this year at the end, but it also seemed to wildly over the top been done elsewhere.. but, I guess it is one way to cut how many people are in the staff and introduce new characters.
Though Who TF would want to work in this accursed place ever again?

This was way more intense and a higher body count! I must say I was on the edge of my seat! They milked it and the first two people shot were surprises. The stand-off with Derrick was intense too. I was pretty sure he'd get shot. It's too bad Dr. Bailey could not get her patient off that floor to save him. Too bad Meredith had the miscarraige. They sure trimmed away most of the extra docs that came from the other hospital in the merger. It looks like the shooter pointed the gun on every main character on the show, except maybe Sloan. I knew Karev would would survive! And it looks like Chief has got his job back now that Derrick is out of commission. Looks like Sloan and Teddy might be back together now that Lexie has said she loves Karev. Unless the surviving Karev still has feelings for Izzie. Oh man, what a soap opera!
As I was saying in another thread, ABC has had a huge body count this season in all the season finales!

These 2 hours were pretty ridiculous and over-the-top, but if you have the clear the decks, this is the most sensational way to do it.
