This ad and rebate certificate was in the Parade Magazine (full page ad) that is included with most (some?) Sunday (06-11-2006) papers.

TERMS
OFFICIAL OFFER CERTIFICATE
GET $5 BACK BY MAIL WHEN YOU: Buy any 2 of the following on DVD:
  • Firewall
  • 16 Blocks
  • Syriana
  • Green Street Hooligans
  • Kiss Kiss Bang Bang
DVD's must be purchased during the "offer period" of 6/6/06 - 7/25/06.
All requests must be received by 8/8/06.

Enclose:
  1. This original, completed Official Offer Certificate (copies not accepted);
  2. Original proof-of-purchase tabs or UPC's from the purchased DVDs.
  3. The sales receipt for the DVDs purchased between 6/6/06 and 7/25/06 with the purchase price clearly circled.
