Best:

The Simpsons - customized chalkboard & couch scenes... I mean, c'mon! An original idea (almost) every time! Oh, and a great score by Danny Elfman.

Futurama - now with more Slurm!

Family Guy - F'n great

King of the Hill - Great "hillbilly" style music, introducing all the characters, etc.

Justice League - Computer Animated superhero's and a great symphonic score!

Batman: The Animated Series - what everyone else said. A fluidly animated title sequence of Batman whoopin' ass. They don't even NEED to print the words "Batman" onscreen for the viewer to know what they were watching.

Enterprise - An inspiring montage of human achievement, and a glimpse at what we can achieve as a species if we learn to learn & accept each other... as a species (and I love the music too).

Star Trek TNG/DS9/Voyager - Great CG/Model work of the great void of space, backed up by a creatively produced symphonic score.

Desperate Housewives - Very Andy Warhol'esque with a cute soundtrack to match.

Worst:

Any reality show - Ooooo Dramatic! Oh wait, maybe not.

Justice League: Unlimited - They took a great opening montage with a symphonic score, and turned it into clips from the CURRENT episode (thereby ruining it for the viewer), with a riffy rock-guitar soundtrack. Blech!

Teen Titans - Oh man, is this ever ANNOYING music! Sounds like the Chipmunks singing, AFTER they sucked down a few helium filled baloons. UGH!

No more come to mind at the moment, but I KNOW there are several that I despise.

Moe.

What? No mention of Monk?

I love the song, and it really gets you kinda in tune with Monk's perspective.

It's the only intro I'll actually watch when I have it recorded, with other shows I just skip them.

Favorites:

Babylon 5 - Different opening sequence and music for each season, yet they are almost all renditions of the one main there. beautiful stuff, more shows should do that. S3 is my favorite, the resolutely martial music signals that everything that the station stood for has gone to hell.

Buffy - While the excellent music remains the same, I like the change of scenes for each season. Angel isn't as good.

E.R - I never watched a single ep of this show, but love the music.

The Practice - Equally urgent and quirky.

Six Feet Under - Classic Newman, love it. Quirky like the show.

Smallville - Surprising, because I typically hate songs, in favor of instrument music.

Law & Order - Simple, classic.

West Wing - I only like the first few seconds.

I am sure there are others.

everyone says American Idol. I've never watched the show but have heard the theme song ad nauseum thanks to all the entertainment programs. I would say that music is better than any of Clay or Ruben's songs

Speaking of Jerry Goldsmith, I'm surprised nobody has yet mentioned one series in particular that has not only a really great title sequence but also some great Goldsmith music to go with it:

Star Trek: The Next Generation

This one is followed up by the two variants of the Star Trek: Deep Space Nine title credits sequences - the first one (seasons 1-3) and the second one (seasons 4-7).

Here's one of the worst title sequences I've seen:

Hunter- I couldn't stand the music! Blech! Hated it.

Long Live DVD!

No one's said X-Files yet? PERFECT intro.
Also, South Park's is great: energetic, and leads right into the episode.

yes, as much as I love SP, I don't like the season 6+ theme as much as I did the first two

I've always been a fan of The Equalizer.

He obviously misinterpreted what it means to "be bullish."
