How do they choose Cover Art for a TV show DVD? I mean seriously... some of them really suck.

I think part of the idea is to have the art reflect the period from which the TV show came. So, for example, the Dick Van Dyke Show DVD cover art reflects tastes of the early 60s, and so on. That's one of the things I love about DVD cover art.

A lot of companies simply use publicity photos for cover art. Take the cover art from seasons 1 & 2 of "Three's Company". Anchor Bay used nothing more than publicity shots for these. Even the "Frasier" and "Cheers" sets used basically cast photos.

Frankly, it makes no difference to me what's on the cover. It's what's inside the package that I'm purchasing.
