HT speaker location


I am installing in ceiling speakers for the side and rear surrounds on my HT system and I am wondering how to figure out the ideal location. The sides will be individual Paradigm SA15R's and the rear will be a single SA15RSM. Should the rear be further back than the sides? How close to the walls can / should I put the sides? Also I have read it may be a good idea to put insulation in the bays before installing the speakers. Any thoughts?
For the mains I am installing Paradigm SA35's. They are going in an outside wall with blown in insulation. Any concerns here? Finally I was planning on using a Parafigm CC590 for the CC as there is a duct preventing me from using an in wall for the center. Does anyone foresee a problem with this. Thanks in advance for your thoughts on this matter.

With a 6.1, the rear center goes in the middle.
