Another option is an older dbx model, like this 1513. They came with removable rack ears. This particular model is switchable between stereo 15-band and mono 31-band (which is the way you’d want to use it).

Parametric EQs are better for subs than the graphic models you're looking for, but you can find them with your requirements. Your best bet is probably a half-rack model like this Symetrix or this ART. I think dbx used to make a half-rack model, too – you might check e-Bay from time to time..

The half-rack models typically have removable rack ears, but the downside is that they usually only have three bands (due to their reduced size). So to adequately equalize your sub you might need two of them daisy-chained.
