Check out the Harmony 670:

It was specially designed for people with TiVOs and other DVR's. All the recording buttons (FF, REW, Pause, etc.) are grouped around the middle for easy access. If there is universal agreement (or near agreement) on any product here at the HTF, it is the Harmony brand of remotes. They are simply years ahead of most (all) remotes out there.
As far as the speaker clarity is concerned, a surround sound setup will allow you to calibrate the sound so that it is balanced and distributed properly between the speakers. Some people find that after calibration, a bump or two in the dB output of the center speaker can make dialog clearer. This is something you can't get with standard TV sound.
