Audyssey ignores any setting prior to running it, so no, you don't need to change anything first.
When you say 'crank it to 54', remember that these numbers are relative and cannot be compared to any other system (unless it is truly set to reference). Yours may be set to reference by Audyssey (some models are, some are not). Either way, I wouldn't worry about it.
Also, TV is not the best source for testing you speakers. TV shows can be really great sounding or really horrible (and everything in between). Use DVDs and Blu-Rays as they are mastered to a set reference.
Music CDs can be all over the board as well....

Thanks David. I'll try the DVD/BluRay route. Most of my efforts have been geared toward TV programs and TV Sports since thats most of my listening experience.
On another note, I have my listening mode now set to PL IIx. It is my understanding that this setting will utilize all 7 speakers for shows that are broadcast in 5.1. I can hear sounds (at times) from 5 speakers but not 7. If a TV program sends certain sounds to the 2 rear surrounds shouldn't I also hear that same moment of sounds in the 4 surrounds?

PLIIx does expand 5.1 to 7.1, but it is not a 1 to 1 relationship. Not everything is sent to the back surrounds. What gets sent and what doesn't is part of Dolby's secret sauce.
I find in my room that 7.1 adds a little to my sound but not a 'night and day' difference from 5.1 (and I have the proper space behind my listening position - something many folks who setup 7.1 do not have). If I had to remove the rear speakers I do not believe I would miss them....

Ideally, in an 5.1 environment, do I want the surround speakers placed immediately to the right and left sides of the listener or behind the listener? And where for the other 2 in an 7.1?

Doesn't the 605 have the same scale as other Onkyo's? That should be -54, or 54 below reference level. I usually run my 876 at a minimum of -40, usually -25 to -30. The volume knob has no bearing on the Audyssey calibration. It would make no difference where you had it set. I think you're just expecting too much from the surrounds, especially on most TV programs. If you could find a THX demo disc somewhere, that would give you a real good idea of how your surrounds are actually functioning. I did notice some gunfire coming from the surrounds on Sons of Anarchy the other night, but for most TV you won't get much.

I mentioned this before, but will again. The best 7.1 sound I've gotten from tv, using DPL IIx, is the Mercedes Benz commercial, when the car is zooming down the roads, then crashes through the glass, on the front of their showroom. There's some good sound coming from the back surrounds, a couple of times, while the car is zooming down the road. It's either been on NBC or Discovery channel, when I've seen it. Have you seen this, and how did it sound to you?
As has been mentioned, you don't normally hear a lot coming from the back surrounds. It's just nice when you do. Also, as was said, when it does, don't expect it to be distracting. Afterall, these are ambient sounds, and not major sound effects.

We get it here at 8:00 pm. You might need to check the time for your area. Probably the same, since you're in NC too. I've noticed a good bit of stuff coming from the rears, all through this program. Especially guitar playing.
Just something else to check your speakers with. Hope all is fine.
