Lost Blu-ray rebate

I bought Lost seasons 1 and 2 through Amazon and I'm wondering whether the enclosed rebate will work for this purchase.  The rebate form says that you need a cash register receipt with store name and dated between 6/16/09 and 6/16/10 and store receipt must be from the same state as the consumer address.
I ordered this online through Amazon on 6/9/09 and I have a copy from my account stating that is was shipped on 6/15 with delivery estimate of 6/18/09.  My packing slip just has the order date of 6/9/09.
Anyone know or has had any luck with this rebate?

I am away traveling so I don't have it in front of me, but when I read the form, I thought it mentioned something about "one rebate per family." So, although I double-dipped on both seasons 1 & 2, I believe I can only get one $20 rebate (NOT $20 apiece.) As far as Amazon goes, that should not be a problem since they have multiple fulfillment centers around the country.

Costco also has all the seasons for $48.99 each, and you get $20 back instantly when you buy two seasons.

^^Most stores offered the same deal during the release week for seasons 1 & 2. Combine that with the $40 mail-in rebate, plus whatever you get from selling or trading in your DVDs, and IMO this becomes quite an affordable upgrade path.
