Most rabies tags have this info on them, and most states do require wearing rabies tags. I know some dogs are good at losing them, and unscrupolous unscrewpuls unscrupolis bad people can take them off, so a microchip is a good idea. Even that isn't foolproof, as there are several different brands and standards of microchips, and most readers are not capable of reading all chips.
