Do I have to destroy my old pulp fiction dvd to get the $5 rebate? (1 Viewer)

I can't find a proof of purchase tab. All I see is the UPC tab underneath the plastic lining. How many people have destroyed their old copy or is there a trick to removing the UPC tab? Thanks

Keep case inserts can easily be removed. Just open the case and fold it back; the lining will lift and you can slide the insert out.
